The application and consent forms are presented here to allow you to apply directly to the Teacher Support Program. The application is combined with a consent form through which you give your consent to abide by the conditions of the TSP. Simply complete the form and press the "Submit" button. You will receive confirmation of the receipt of your application on screen.
All data entered on these pages will be used by the TSP in compiling statistics relating to teacher stress, burnout, and retention. The TSP adheres to strict confidentiality. Names will not be associated with specific data.
Once you submit your application you will have immediate access to the website through the participant button (above). Your username will be your last name and your password will be the last 4 digits of your SSN.
